docs(remediation): bank D-56 — the lane idled four days on a verdict that had already landed

Round-4 verdict id 73 (NO-GO @ fbb61912) landed 2026-08-01T18:12Z; remediation was pushed the same evening; the round-5 re-review dispatch was lost at the rotation seam and 'awaiting rev-974 verdict' was carried as fact for four days. Provider re-measured, round 5 dispatched (durable PR comment + verified tmux), board corrected. General form owned by Mos: a waiting-on-X state line decays like any other measurement and must be re-measured at rotation.
