The standing merge bar required an exact-head APPROVE RoR; the only independent verdict remained literal REQUEST CHANGES. The carve ruling and APPROVE-context comment scoped the residual but did not replace reviewer verdict polarity, so merging #852 lacked the required affirmative review artifact. Do not reverse linear main or misstate shipped-core quality: core findings/CI passed and #850 preserves the residual. Record this as a process incident/tooling-policy gap, require a reviewer-issued reconciled verdict after any carve+ship BEFORE future merge, and do not treat coordinator scope rulings as transferable approval. wp1937 push/ci remains pending; publish is green. FCM-M5-001/#848 hold unchanged.
