PR #866 round-12 independent CODE verdict: APPROVE at exact head 8ac7e70f0db6458f824833a438fb69a8ea759db5. CI1966 terminal-green at the same SHA. Reject-all-anchor handling closes the duplicate-anchor fail-open; targeted 16-case matrix and independent 542-case anchor/property sweep found 0 fail-open/divergent emissions. Valid single anchors are intentionally conservative fallback failures while PyYAML acceptance remains asserted; this is a legitimate behavior adaptation, not assertion weakening. Non-specific ! tag and quote-aware folding regressions remain green. DO NOT MERGE: Gate-16 provider-visible reviewer independence, queue state=unknown, stale PR body, and absent Mos named-executor clearance remain independent holds.
