Provider has a REVIEW-OF-RECORD: APPROVE comment naming this head, but no native review records. Exact diff audit found blockers: issue-comment.sh read-back searches all historical comments by body only, so a pre-existing identical body can falsely validate a silent no-op (no pre-write boundary/timestamp or created-record ID); pr-review.sh approve/reject state remains explicitly trusted from tea exit zero, contrary to never-trust-exit-zero scope. Queue remains literal state=unknown. Do not merge; needs exact-head remediation + fresh independent RoR/CI, and named Mos executor authorization.
