Scope: fixes the tea 0.11.1 silent-no-op comment bug in issue-comment.sh (top-level tea comment form) plus a bounded read-back that fails closed, and adds bounded review-state read-back to pr-review.sh approve and reject (removing the exit-zero trust). pr-review.sh comment path was already fixed by #812/#835.
6-check status:
1. PR open, not merged, mergeable, base main: PASS.
2. CI TERMINAL-GREEN at exact head: PASS — Woodpecker 1955 success, all 8 steps OK, finished 232856Z, commit 10fdd49e.
3. Independent reviewer (author not equal builder) durable RoR at exact head, read back: PASS — Gitea comment 18470, REVIEW-OF-RECORD APPROVE, names head 10fdd49e, unedited, I read it back myself.
4. reviewed-SHA equals CI-SHA equals head: PASS — all 10fdd49e.
5. body Closes #865: PASS.
6. queue-guard: state=unknown — the guard returns literal unknown on this Gitea platform (you and homelab have both seen this baseline); no running or queued main-push pipeline observed (1955 was the PR build, success; latest main builds all terminal). I flag this honestly as the one non-clean item; it reads as a platform introspection gap, not an observed conflicting job.
REVIEW HISTORY (full transparency): first head a27f1fa7 got a web1 reviewer APPROVE (RoR c18465), but homelab ran an independent exact-head audit and caught a real BLOCKER my first reviewer missed — issue-comment.sh read-back matched body-only across all history, so a pre-existing identical body could falsely validate a silent no-op. I ruled NO-GO, routed remediation (id-bounded read-backs), and required a fresh independent review plus fresh CI at the new head. The current APPROVE (18470) and CI 1955 are at the remediated head 10fdd49e; prior c18465 and CI 1954 are void. Homelab was invited to re-audit the remediated head in parallel and has not responded (non-blocking; check 3 is satisfied by the fresh independent review).
RECOMMENDATION: GO to merge (squash), which closes #865. Merge and the named-executor provenance stamp are yours. Name the executor and I stand ready to verify post-merge state. I will NOT merge myself.
Out-of-scope follow-up (do not action now): issue-close.sh, issue-reopen.sh, pr-close.sh still carry the same broken tea-comment form — captured to OpenBrain for a separate follow-up issue.
