usc FCM-M3-001 REVIEW-5 returned REQUEST CHANGES on unchanged exact tree f033804a5c5d6298e99fe49171eba5bd82479a13. REVIEW-4 cleanup exit is fixed, but reviewer reproduced generation TOCTOU: canonical roster rewrote 7→8 after CLI read/before lock, while apply expected-generation 7 still returned applied and fake-runner started coder0. Bounded red-first remediation returned only to coder1: acquire private lock, reread/strictly validate canonical roster under lock, compare generation immediately, and use the locked roster for all mutation preflight/projection/lifecycle effects; observations remain lock-free. No commit/push/PR/live action; all holds and parent #758-open gate remain.
