mosaic gateway doctor ignores mosaic.config.json written by the CLI itself #1393

On a standalone-tier install whose mosaic.config.json was written by the CLI, doctor reports "Tier: local (auto-detected)", skips the postgres/valkey/pgvector probes entirely, and reports Overall GREEN.

Doctor must load the same config precedence chain as the gateway (mosaic.config.json over env) before choosing which probes to run.

Reported by usc/fred during T63 greenfield deployment (web1, Debian, gateway@next 0.0.7-next.2636). Filed by orch-01. Tag: T63 batch 2.
