Ledger sync m4

Sole-writer ledger update recording merged reality:

  • TESS-M5-003 → done — PR #746 (documentation hard gate, 7 docs-only files) MERGED by Mos → main bc8016c8. Reviewer VERIFIED APPROVE at exact head 25b9d642 (Gitea 17170); CI 1786 success.
  • Prior ledger sync m3 PR #745 MERGED → main e72388b2 (recorded M5-001 + M5-002 done).

Docs-only (docs/tess/TASKS.md). Tracking-trio otherwise untouched. command-authz byte-identical a9f829e7. Mission issue #706 preserved OPEN.
