docs(remediation): bank D-56 — the lane idled four days on a verdict that had already landed

Round-4 verdict id 73 (NO-GO @ fbb61912) landed 2026-08-01T18:12Z; remediation was pushed the same evening; the round-5 re-review dispatch was lost at the rotation seam and 'awaiting rev-974 verdict' was carried as fact for four days. Provider re-measured, round 5 dispatched (durable PR comment + verified tmux), board corrected. General form owned by Mos: a waiting-on-X state line decays like any other measurement and must be re-measured at rotation.

evening**. Each blocker has a corresponding change (history anchor excised and held at the RM-60

being re-read — **and a rotation is exactly when it must be re-measured, because the successor
inherits the belief with no way to date it.**
Two requirements fall out:
1. **A dispatch is not "sent" until receipt is observed** — the charter's message-delivery clause,
applied to review dispatches specifically. `agent-send` "queued" is not "processed". Every review
dispatch MUST leave a **durable provider-side record** (PR comment) in addition to the tmux
message, so a lost send is detectable by anyone from the artifact rather than from memory.
Binding on this seat's process now; on the coordinator daemon (RM-58/RM-63 class) when it exists.
2. **Rehydration briefs must mark every inherited "blocked on X" as PROVENANCE, not fact** — the
continuity file already says this for SHAs; it now explicitly covers waiting-state lines, which
are the same class one level up.
**Fix applied same tick:** round-5 re-review dispatched to `rev-974` at `e910a45a` — durable PR
comment plus verified tmux delivery — and the board/continuity corrected.
